COVID-19 Vaccine for UPHS Employees

Preparing for your appointment

UPHS employees can schedule a COVID vaccination at an Occupational Medicine location. Appointments can be scheduled either during or outside of your work shift. Vaccine recipients will not be required to stay for a post-dose 15-minute observation, unless there is a history of severe allergic reactions, in which a 30-minute observation time is required. UPHS paid employees will be paid for the time required for the appointment, including any observation period. Before scheduling your appointment during a work shift, please coordinate with your manager to minimize impact on patient care and operations.

Before coming to your scheduled appointment:

• Print out your employee barcoded Moderna vaccine consent form to bring to the appointment
• Review the COVID-19 Vaccine Information Statement (VIS).

By scheduling and appearing for a vaccine appointment, you are consenting to receive care at Penn Medicine. The following documents are relevant to this care:
